Disability Case Management enriches and empowers individuals with disabilities, their families and caregivers through affordable, high-quality professional services, education, and advocacy.

Our services focus on helping each person become as independent and fulfilled as they choose to be. We serve the following:

Trasition Aged Youth (14-22)

Adults: (22+)

Parents and Caregivers

We offer the following services:

  • Life Planning/Life Coaching: Budgeting, Reaching out to Resources, preparing for life changes
  • Assistance applying for Financial Assistance Programs: CEAP, SNAP, Medicaid
  • Transition Audits: Planning for the future
  • Reserach and Resources: Connect to resources through research
  • Home Visits/Check Ins: In Home Applications, Deliver paperwork, food bags
  • SSI/SSDI Benefits: Application prep and submission.


As a United Way agency, our Coaching and Case Management services, information and referrals are available to all residents in the Greater Houston Community. Please complete this form and the Alexander JFS team will be in touch within two business days.
